"while we were away"

  Away from what exactly?
  Where ever they were they would have been able to receive an E-mail of the 
design and said proceed or not . In this day and age you cannot "go away" 
anywhere. Even if they did mange to go somewhere with out communications to 
the outside World , they still should have made sure that they were in a 
position to view the art-work before the go-ahead was given to print it.
  Simply blaming the manager and saying that "they were away" is simply no 
excuse. Its their album and they should have had an input to its artwork, 
rather than leaving in the hands of the manager and then blaming him when 
its crap
