Sounds to me like a mistake at the pressing plant.
Someone forgot to switch the artwork, too many cds pressed with promo artwork, Zuma probably got 'em cheap and thought

"Fuck 'em. that'll do them. they'd buy a turd with Killing Joke written on it at the moment".
